The main goal for the project is to create intake forms for the Omega Pediatrics website in order to bring in new patients. The forms should be created using Jotforms and should have logic implemented such that the form fields presented to the patients will depend on the age of the patient.

Deliverables


- Creation of intake forms using Jotforms


- Testing of the forms to ensure they function as intended


- Implementation of the forms for patients of Omega Pediatrics

Free and paid software is provided for the students to use for the work being done. They will also get support from team members who will supervise the project and meet with them regularly.
